Training debt is the reason most physician files fail, and the three ways a lender can count it produce wildly different answers on the same borrower.
Three rules, one balance
The same borrower with the same loans gets three different monthly obligations depending on which rulebook applies.

Why this bites harder in California
Because of what the house costs. A $3,000 phantom debt is survivable against a $400,000 purchase. Against a Bay Area typical value over $1.1M, or San Jose's $1,527,731, it is usually the single line that ends the file.

California runs two physician loan-repayment programmes, and both are closed to new applications. The Steven M. Thompson Physician Corps programme — up to $105,000 for three years of service — reopens May 2027. CA-SLRP reopens July 2027. (HCAI, read 2026-10-05.)
So for a 2026 purchase the loan-repayment route is simply unavailable, and the balance underwrites as it stands. The full status.
There is a second reason this matters even once the windows reopen: an award you have not been granted is not income a lender can use, and a balance that has not yet been paid down is still a balance.
What "documented" means in practice
The payment has to be evidenced, not asserted. In practice that means a current statement or servicer documentation showing the actual scheduled payment under your plan.
Two situations worth raising early rather than late:
- A zero payment. Treatment of a documented $0 IDR payment varies by programme and by rulebook, and it is the single most common thing to get wrong by assumption. Ask before you plan around it.
- A plan change mid-file. Recertification can move the number. If your plan is due to recertify between application and closing, say so at the start.

Public service forgiveness
Many California residents and fellows train at non-profit academic systems and are building toward Public Service Loan Forgiveness. That is a long-horizon plan and a mortgage is a near-horizon decision — forgiveness at year ten does not reduce the payment a lender counts today.
What it does change is the shape of the right advice: if you are on an income-driven plan precisely because you are pursuing forgiveness, then a programme that counts that documented payment rather than 1% of the balance is not a convenience, it is the difference between buying and renting.
